    • All communication between the Obligee and the Contractor about the project

    • Detailed repair estimate/quotation for the affected building showing quantity and unit cost per item of work prepared by duly licensed Civil Engineer or Architect or Contractor to place the building in the same state/condition as before the loss
    • Floor Plans with Specifications of affected property
    • Lot Plan and Vicinity Plan
  • Contents Claim
    • List of the affected household contents to include quantity, description, year of acquisition and replacement cost, and/or repair cost
    • Acquisition documents of the affected items such as sales invoices, purchase receipts, etc. If not available, please provide old photographs of these affected items contained inside the premises
    • Fixed asset schedule
  • Stocks Claim
    • A detailed inventory of the articles damaged and undamaged stocks, showing the cost price of each, extent of loss, and salvage value
    • Proof of costing
    • Latest inventory of merchandise filed with the BIR
    • Copies of Income Tax Returns and Audited Financial Statement for the last three (3) years filed with the BIR.

    • Purchase / Sales Invoices

  • Machineries and Equipment
    • A detailed list of the articles damaged or destroyed showing the cost price of each and the date acquired
    • records, invoices, receipts, if any.

  • Submit only certified true copies except as indicated below
